Objective:
As the end of their school career approaches students must think about their
lives without the structure that school has provided. Students need
to consider their health and leisure, how they will fit into the community,
and have a plan for what will happen when they leave the public school system.
Requirements:
All Grade 12 students are required to complete the following:
Healthy Eating Plan (includes a one week diet log with reflections)
Stress Management Plan (in package)
Emotional Health Plan (in package)
Plan to complete DPA (in package)
30 hours of work or volunteer experience
Resume (updated and current)
Covering Letter (generic)
Application form (generic, and included in package)
Reference Letter (from teacher or work supervisor)
Daily Physical Activity (logged 150 minutes per week)
Plans for post-secondary
Application to a post-secondary institution of learning
Interview highlighting accomplishments and discussing future plans
Expectations:
Students must turn in the Graduation Transition booklet to Ms. Chalmers
in room 234 by the following dates.

Evaluation:
Graduations Transitions is not a graded class. The end mark will read Requirements
Met or “RM”. For each term of the report card
the student will either receive a 100% or an I.
Ms. Chalmers in room 234 is available for support and guidance.
